Neither sitting nor standing! Is it a big misfortune for a young man to get "hemorrhoids"? How to choose hemorrhoid medicines and understand them in one go

Is it a big misfortune for a young man to get “hemorrhoids”? How to treat hemorrhoids if they are discovered?

Have you ever had hemorrhoids? Have you ever experienced the pain of not being able to sit or stand? Dr. Lan Yuanci, an attending physician at the Department of Colorectal Surgery at Taiwan Ministry of Health and Welfare, Food and Drug Administration and Taipei Veterans General Hospital, pointed out that hemorrhoids are vascular tissue located under the skin and submucosa around the anus. Due to factors such as long-term improper defecation and aging, hemorrhoids It leads to blood vessel congestion and expansion or connective tissue relaxation, causing bleeding, lumps, prolapse, itching and other symptoms. Modern people sit for long periods of time and do not exercise. They eat less fruits and vegetables during meals. Their defecation is not smooth and their mobile phones keep sliding when going to the toilet. This has led to many people suffering from hemorrhoids as teenagers, but they are too shy to seek medical treatment.

“Generally speaking, asymptomatic hemorrhoids do not require treatment!” Dr. Lan Yuanci said that early hemorrhoids have mild symptoms and can be relieved with conservative treatment and medication. Conservative treatment involves improving diet and defecation habits, increasing the intake of fruits and vegetables and adequate fluids, avoiding prolonged sitting and improper exertion during defecation, using warm water sitz baths to improve anal discomfort, and developing the habit of regular exercise.

In terms of drug treatment, there are three common categories: ointments, suppositories, and oral drugs. Among them, ointments and suppositories are mostly compound formulas. Suppositories are used for internal hemorrhoids, and ointments are used for external hemorrhoids. Common ingredients are as follows:

  1. Local anesthetics (Lidocaine, Benzocaine, Cinchocaine): can relieve pain.

  2. Antihistamine (Diphenhydramine): can relieve itching.

  3. Vasoconstrictor (Phenylephrine): can reduce swelling.

  4. Steroids (Hydrocortisone, Fluocortolone): can reduce swelling and inflammation.

  5. Skin astringents (Zinc Oxide, Kaolin, Aluminum Acetate) lubricate the skin and lock in moisture.

  6. Bactericides (Bismuth Subgallat, Polycresolsulfonate, Resorcinol): antibacterial, anti-inflammatory, and promote wound healing.

  7. Allantoin: Can repair tissues.

  8. Vitamin E acetate (Tocopherol Acetate) can improve blood circulation in the affected area.

  9. Menthol: It has a cooling effect and can relieve pain.

Dr. Lan Yuanci said that patients should pay attention to when using hemorrhoid ointment. If allergies or contact dermatitis occur, they should stop taking the medicine immediately. If it contains steroid ingredients, avoid long-term use to avoid side effects such as atrophy and thinning of the skin and mucous membranes.

Oral medicine helps improve bleeding and itching. Doctor’s appeal: Don’t be shy to seek medical treatment if you have hemorrhoids.

In addition, Dr. Lan Yuanci also pointed out that oral medicines such as non-steroidal anti-inflammatory analgesics (NSAID) and hemostatic drugs (tranexamic acid) are commonly used to improve the pain and bleeding caused by hemorrhoids; laxatives and stool softeners can improve constipation and reduce constipation. Hemorrhoids attacks caused by improper constipation; phlebotonics (flavonoid plant extracts), such as Daflon and Diosmin, are helpful in improving bleeding and itching caused by hemorrhoids.

Dr. Lan Yuanci reminds that if conservative treatment and drug treatment are not effective, or recurrences occur after treatment, further medical treatment should be conducted to rule out the possibility of anal fissure, fistula, abscess, colorectal cancer or inflammatory bowel disease. The Taiwan Food and Drug Administration advises that colonoscopy be performed when necessary to find out the root cause, and do not avoid it and delay treatment.
